Job Description
Temporary Full Time M-F Days Position Summary: Social Workers are entrusted to provide pro-active social work for high risk patients to optimize the patient/family experience in potentially life-altering situations. They are an integral part of a multidisciplinary team whose purpose is to provide a personalized, safe discharge for our patients. Required Qualifications: Graduate of an accredited bachelor or master school of social work. Current NM licensure Excellent customer service skills Preferred Qualifications: Computer experience One year's experience in a health care setting Duties and Responsibilities: Interview patients and/or family members to obtain information about social, emotional, and financial factors which impact health status. Assess needs for discharge planning and continuing care or resource support following discharge. Respond to referrals from patients, families, physicians, hospital staff, and community agencies for family violence, adjustment to disability/illness, complex discharge planning and behavior management. Participate in interdisciplinary team approach to develop appropriate post-hospital plans for continuing care. Discuss Advance Directives with patient/family and obtain a copy of the medical record. Maintain ability to counsel patients and families on end-of-live issues. Demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's requirements relative to his/her age specific needs. Maintain adequate working knowledge of local resources. Maintain up-to-date knowledge of entitlement programs, eligibility requirements and benefits for State and Federal Assistance programs. Maintain cultural competence in assessing and treating patients from the diverse cultures of the Four Corners. Demonstrate knowledge and skill in providing for patient legal and ethical concerns. Provide mental health and substance abuse screening and referral. Provide crisis intervention and/or short term counseling, Serve on hospital committees, task forces and work groups as deemed appropriate. Enhance professional growth and development through participate in education programs, review of current literature and attendance at staff and in-service meetings. Assist in reviewing an updating policies and procedures to maintain excellent case management services. Each employee is responsible for implementing SJRMC's Service Standards into their daily work.
