Job Description
Full time Monday-Friday Days Position Summary: This position is entrusted with the responsibility to support the staff of the case management department. This includes staff supervision of counselors, RN and social work case managers involved in discharge planning, utilization review and social work services, as well as communicating with physicians, third party payors, patients/guardians, and outside resources to ensure continuity of care for the patients of San Juan Regional Medical Center. Required Qualifications: Graduate of an accredited school with either a Masters of Social Work or a RN degree.Current NM licensureMinimum of 4 years experience in a health care setting, with a minimum of two year in an acute care setting.Excellent customer service skills Preferred Qualifications: Computer skillsIf RN, Bachelor's level or higherOne year managerial experienceCase Management Certification Duties and Responsibilities: 1. Supervise the department in obtaining information about social, emotional, and financial factors which impact health status, and to begin discharge planning upon admission.2. Maintain good working knowledge of local resources. Maintain up-to-date knowledge of entitlements, eligibility requirements and benefits for State and Federal programs, including exempt Medicaid, SSI, and SSDI. 3. Maintain up-to-date knowledge of CMS/JCAHO requirements for department and ensure that department maintains compliance to these requirements.4. Develop and maintain budget for department. Responsible for monthly financial responsibility report.5. Promote and maintain good working relationships with human services programs in the Four Corners area including, but not limited to, nursing homes, State agencies, home health care agencies, durable medical equipment providers, emergency shelters.6. Maintain appropriate licensure and adhere to ethical and professional standards as put forth by professional organization.7. Provide oversight to the Pastoral Care department.8.
