Job Description

Full time varies Days Position Summary: Work with patient and clinical team to provide personalized nutrition recommendations to promote patient healing. Educate patient and family with practical dietary suggestions, being mindful of patient capacity for growth in the context of their cultural and emotional environment. BS Dietetics; Registered Dietitian credentials, Licensed State of New Mexico? Ability to work individually and in a team.? Self-directed? Ability to prioritize? Excellent verbal and written communication skills? Ability to instruct and coach staff? Excellent customer service skills Duties and Responsibilities: ? Provide nutrition education to inpatient and outpatient clients at SJRMC, SJRRH and CTC to enable selfmanagement of medical nutrition therapy: Provide education to patients/caretakers in a manner that is understandable to them, and enables them to meet their desired outcomes of medical nutrition therapy. Document education in inpatient medical record within 24 hours of service. Provide documentation to physician offices within one week for outpatient referrals.? Complete nutrition assessments on patients at nutritional risk at SJRMC and SJRRH. Make recommendations for medical nutrition therapy: Complete assessments within timeframe as per policy. Notify physician and other professional staff of concerns/recommendations. Enter assessment into medical record.? Follow-up evaluation for nutrition risk patients: Re-evaluate and review care plan within at least 1 week of initialassessment and/or prn. Document.? Respond to telephone inquiries re: nutrition: Respond to phone inquiries re: nutrition. If by phone message, attempt return call within 48 hours of receipt of call.? Conduct nutrition education classes for patients and in-house staff. Examples: cardiac, pulmonary and diabetes classes for patients; graduate nursing students and physician CME: Design and present classes with most current nutrition information. Level information to be audience-appropriate.? Community nutrition presentations: Develop and present timely nutrition topics that are of interest to the community. Coordinate with marketing if appropriate. Complete community report form.? Participate on medical center committees. Serve as an educational resource to other staff: Attend committee meetings as an active participant, and report progress to manager.? Enhance professional growth and development to maintain current working knowledge, ADA registration and state licensure: Assure own CEU accumulation, following portfolio guidelines of ADA. Maintain current registration and license.? Department education: Attend department in-services and meetings. Participate in QA/PI programs when assigned.? Healthstream: All annual Heathstream modules 100% completed.? Each employee is responsible for implementing SJRMC's Service Standards into their daily work. Safety --> Courtesy --> Effectiveness --> Stewardship ? Other duties as assigned: Demonstrates willingness and follows through with other duties requested. Physical Demands and Environmental Work Conditions: ? Mostly paperwork. Extensive walking, sitting, standing required.
